New Member Application Click on the "new agency application" tab and find out what you need to do to become a member of CBH. Make sure you download other pertinent documents and the criteria for membership page under the section called "surveys and other membership docs". Agencies requesting membership to CBH must forward a signed copy of the criteria for membership, the new member application, an agency brochure or other document describing services provided and the most recent audited financial statement. This info will be presented to the Board for consideration.
Renewal for Current Members Each June we send to our existing membership the renewal application. This includes the actual renewal form and the member profile document. Renewing members are also required to forward the most recent audited financial statements. Agencies renewing their membership should forward to the CBH office the renewal form, member profile document, audited financial statement and a check for 1/4 the amount of your annual dues (as calculated from the renewal form).
